1- Copy of IC
2-Copy of Sijil Perkahwinan
3-HIV result test
4- Letter from employer stating that you're still single!
5-Letter stating that you're a mastautin (where actual home address are not same as in IC)
These documents are needed only for a bachelor. If let say the person is a 'duda' or marrying for the second time, it would require other forms.
